1.Acknowledgment of Physical Activity Risk

I understand that participation in personal training, exercise programs, and physical fitness activities involves inherent risks, including muscle strain, joint injury, cardiovascular complications, dizziness, fainting, or accidental injury from equipment or movement. I voluntarily choose to participate and acknowledge these risks.

2.Health Status Disclosure & Responsibility

I certify that I am physically capable of participating or that I have disclosed all medical conditions to my trainer. I understand trainers are not medical professionals and I am responsible for seeking medical clearance if needed.

3.Voluntary Participation & Right to Stop

My participation is voluntary. I may stop or modify exercises at any time and will notify my trainer if I feel pain, discomfort, or unusual symptoms.

4.Assumption of Risk (Oklahoma Law)

Under Oklahoma law, participants assume inherent risks of physical activity. I voluntarily assume all known and unknown risks, including those arising from ordinary negligence. This does not apply to gross negligence, willful misconduct, or intentional harm.

5.Release & Waiver of Liability

I release and waive liability against D&N BODY FITNESS, its owners, trainers, staff, and agents for claims arising from participation, including ordinary negligence, to the fullest extent permitted by Oklahoma law.

I agree to indemnify and hold harmless D&N BODY FITNESS from claims or damages arising from my participation or failure to disclose health information.

7.Photography & Media Release (Optional)

I grant permission for photos or videos taken during training to be used for promotional purposes.

9.Governing Law & Venue

This Agreement is governed by Oklahoma law. Any legal action shall be brought in Oklahoma courts.
